To ensure a lifetime of good health for you and your family, our Family Medicine providers will:

Making an Appointment
Our Family Practice Clinics offer appointments all day on Monday-Friday, with some evening appointments available. To schedule an appointment, please call 505.994.7397.
For your appointment: Please remember to bring any relevant medical records and a current list of all medications. If you are a new patient, you can download and fill out the New Patient History Form before your first appointment.
